Are you qualified for
CDPAS / CDPAP?

If you are currently receiving home care services and you have Medicaid, you can still transition into CDPAP \ CDPAS. In order to determine eligibility, you will need to be assessed by a Managed Long-Term Care (MLTC) representative. Remember, a CDPAP \ CDPAS recipient must be willing and able to make informed decisions about the management and services they receive. They may also choose a legal guardian or a relative make these decisions for them. This means the relative or guardian will be responsible for recruiting, hiring and training the caregiver.

Once you’ve been deemed eligible for home care, you will have the option of enrolling in the CDPAP \ CDPAS and a plan of care will be offered by the MLTC to include the amount of hours approved.

Do I need Certification?

There is no license or certification required to become a CDPAP / CDPAS
Personal Assistant. The client has the option to choose anyone they like to
become a CDPAP / CDPAS Personal Assistant, even without having prior
education, experience or certification in healthcare (except: spouse, parent of child under 21 years old, designated representative).

We make the enrollment process simple and efficient. Once signed up and trained, the Personal Assistant is able to assist the patient in all areas required. Since this program is self-directing, the Personal Assistant is able to assist the patient in the same way as a nurse would, including oxygen care, medication management or providing daily injections.

If you’re ready to begin the application process, please give us a call at 718-
972-2500 and an enrollment specialist will help walk you through the process.
