Are you qualified for
CDPAS / CDPAP?
If you are currently receiving home care services and you have Medicaid, you can still transition into CDPAP \ CDPAS. In order to determine eligibility, you will need to be assessed by a Managed Long-Term Care (MLTC) representative. Remember, a CDPAP \ CDPAS recipient must be willing and able to make informed decisions about the management and services they receive. They may also choose a legal guardian or a relative make these decisions for them. This means the relative or guardian will be responsible for recruiting, hiring and training the caregiver.
Once you’ve been deemed eligible for home care, you will have the option of enrolling in the CDPAP \ CDPAS and a plan of care will be offered by the MLTC to include the amount of hours approved.
